14 hours per week | £10,993 per annum (£15.10 per hour)

Are you a compassionate, confident and organised person who wants to make a genuine difference to people living with dementia and those who care for them?
Memory Matters CIC is looking for a Dementia Advisor to join our team in the North Cornwall coast area.
This is a rewarding opportunity to provide trusted information, advice, emotional support and advocacy to people affected by dementia, helping them understand their options, access the right services and feel more confident and in control.
About the role
You will work directly with people living with dementia and carers, providing personalised support through face-to-face meetings, telephone calls, virtual appointments and written communication.
You will:
Manage a caseload and provide personalised, ongoing support.
Assess people's needs and develop appropriate support plans.
Provide information, advice, emotional support and signposting.
Help people access health, social care and community services.
Build strong relationships with GPs, memory clinics, social care and voluntary sector organisations.
Maintain accurate and confidential records.
Promote inclusion and help reduce barriers to accessing support.
Contribute to community engagement, service development and outcome reporting.
Identify and respond appropriately to safeguarding concerns.
Support volunteers and community/group activities where required.
What we're looking for
You don't necessarily need to have worked specifically in dementia services. Transferable experience is welcome, and we will support your learning and development.
We're looking for someone who can:
Build trusting relationships with people from diverse backgrounds.
Listen, understand individual circumstances and respond appropriately.
Communicate clearly and with compassion.
Manage a caseload, prioritise effectively and work independently.
Build positive relationships with professionals and partner organisations.
Keep accurate records and maintain confidentiality.
Use IT confidently, including databases, Microsoft Office, email and virtual communication platforms.
Work collaboratively as part of a supportive team.
Demonstrate a strong commitment to equality, diversity and inclusion.
You must have a full UK driving licence and access to a car, as the role involves travelling across the North Coast Cornwall area.
Knowledge of dementia, caring, health and social care, Mental Capacity or safeguarding would be advantageous, but training will be provided.
